English-medium doesn't mean 'no Chinese'

A frequent surprise for students on English-taught MBBS programmes in China is that they still need Chinese. Lectures and theory may be in English, but the clinical years are different.

During hospital rounds, history-taking and clerkships, you work with local patients and staff who communicate in Chinese. Programmes therefore commonly build in Chinese-language study, often measured in HSK levels, so students can function on the wards.

Why Chinese matters in the clinical years

Medicine is learned at the bedside. The clinical clerkship — examining patients, taking histories, presenting cases — is where a doctor is actually formed, and in China that happens largely in Chinese with local patients.

Because of this, many universities expect a working level of Chinese by the time clinical training begins, and may set an HSK level as the benchmark. The exact level and the stage at which it is required are decided by each institution.

Understanding this early matters, because your ability to learn effectively in the clinical years is exactly what the later India-side screening exam tests.

The HSK, briefly

The HSK (Hanyu Shuiping Kaoshi) is China's official standardised Chinese-proficiency test for non-native speakers, organised through Chinese Testing International. It is widely used by universities to measure language readiness.

Which HSK level a medical programme requires, and by when, varies from one university to another. Do not assume a single figure applies everywhere.

How this interacts with NMC's rules

On the India side, the NMC Foreign Medical Graduate conditions expect the medium of instruction to be English and the course plus clinical training to be completed at a single institution. Chinese-language needs on the ward sit alongside these conditions, not in place of them.

What you must confirm is that the programme you choose still satisfies the NMC conditions — English medium of instruction, single-institution clinical training — while also equipping you with the Chinese needed for real clinical learning. The two are compatible, but only if the programme is structured properly.

What to verify before you enrol

Because provinces and universities differ, turn the language question into a short checklist and get each answer from an official source, not an agent.

Start with the programme’s authorisation. Chinese authorities designate which universities may teach undergraduate clinical medicine in English to international students, and a university outside that designation is not permitted to recruit international students onto an English-taught clinical medicine programme or to issue an English-language degree certificate marked MBBS. The designation is reviewed and republished by the Ministry of Education, so confirm your target university’s current status on the official Ministry of Education source before you accept an offer — an English-taught medical offer from a university outside that designation is a warning sign.

If a recruiter cannot answer these from the university’s own materials — or claims “no language needed, guaranteed” — treat that as a warning sign and verify independently.
